BHS Student Identifying Information

Please complete this form to alert teachers of any identifying information (i.e. name and/or pronouns) that differs in Infinite Campus from how you identify in school.

BHS Guest Pass Form For Dances

Students in good standing at other schools are welcome to attend with a BHS student. Middle school students or persons over the age of 20 are not allowed. To bring a guest from another school you must have your guest fill out this form.

This form must be returned by 12:00pm the Thursday BEFORE the event.

BHS Parking Ticket Appeal Form

Seniors may appeal parking tickets with valid and sufficient documentation.  Only one appeal may be approved per student per senior year. 

Appeals MUST be received in the Main Office within TWO WEEKS of citation - no exceptions.
